HOW TO
FILL OUT A LOG PAGE
The numbers refer to the numbers on the
sample log page.
1.
Enter the dateMonth Day Year
2. How many miles did you drive?
3. What vehicle numbers did you drive and pull?
4. What is the name of the Company you work for?
5. Sign your full legal signature. Just like your paycheck. If you cant read your
signature print your last name.
6. What city and state is their main office located in?
7. Print your co-drivers name. If no co-driver then print none.
8. ( Also known as line 1 ) Draw a horizontal line between the times you were off duty.
9. ( Also known as line 2 ) Draw a horizontal line between the times you were in the
sleeper berth.
10. ( Also known as line 3 ) Draw a horizontal line between the times you were
at the controls of your vehicle.
11. ( Also known as line 4 ) Draw a horizontal line between the times you were
on duty but not driving.
Then draw vertical lines to connect the horizontal lines. See the sample.
12. Enter a number to indicate the load carried. BL#, Shippers #, PRO#, etc.
or enter the shipper and the commodity.
13. Add and enter the total hours you spent off duty.
14. Add and enter the total hours you spent in the sleeper berth.
15. Add and enter the total hours you spent at the controls of your vehicle
16. Add and enter the total hours you spent on duty but not driving.
17. Add all the hours it must equal 24
18. Recap. Not always used. Your company will instruct you.
19. Enter your driving hours total for the day.
20. Enter any time that was in violation of the rules. (See the rules on this
page ) Also indicate in the remarks (section 25) any violation time.
21. Enter the total of lines 3&4. This becomes your total on duty hours.
22. Enter figures from Column A from summary sheet.
23. Enter figures from Column B from summary sheet.
24. Enter figures from Column C from summary sheet.
25. Place a vertical line from each change of duty, and indicate on this line
where the change took place with a city and state. (Do not abbreviate the name of the
city.) Also indicate under the line the reason for the change of duty. (e.g. Dispatch
Pre trip inspection Lunch Loading Unloading Tire repair
Off duty, etc. ( See the definitions on page 4 )
